Advanced Care Partners is seeking a Licensed Practical Nurse (LPN) to provide one-on-one care for medically fragile children and adults in home and school settings. This role offers the opportunity to deliver high-quality, individualized care in a more focused and meaningful environment.
Compensation & Benefits
- Base Pay: $35/hour
- Opportunity to earn up to $43/hour with applicable differentials:
- Trach: +$2/hour
- Vent: +$3/hour
- Nights (7pm–7am): +$2/hour
- Weekends: +$3/hour
- Holidays: +$3/hour
- Weekly pay

Key Responsibilities
- Deliver skilled nursing care, including G-tubes, tracheostomies, ventilators, and central lines
- Monitor patient condition and respond appropriately to changes
- Educate patients and families on care plans and procedures
- Collaborate with interdisciplinary team members to ensure optimal outcomes
Qualifications
- Active LPN license in Georgia or Nurse Licensure Compact
- Current CPR certification (online-only certifications not accepted)
- Minimum of two years of experience preferred
- Valid PPD skin test
Physical Requirements of the Role
- Ability to stand or sit for extended periods (up to 12 hours)
- Ability to bend, twist, kneel, and assist with patient mobility
- Ability to lift, push, or pull up to 50 pounds or more
- Manual dexterity for clinical tasks
