This position will be based in Ulster county, NY for the first 90 days and the schedule will be Mon-Fri, 8a-4:30p, with 1-2 Saturday shifts.
After the first 90 days, this role will be FULLY REMOTE and the schedule will be either 9:30a-6p or 11:30a-8p with rotating Saturdays 8a-4:30p (every 3-6 weeks).
This position supports a healthcare call center by triaging patients and providing clinical guidance to ensure appropriate care and efficient coordination with providers and care teams.
Responsibilities
Triage patient calls to assess medical needs and recommend appropriate levels of care
Coordinate with physicians and care teams to support patient care and follow-up
Schedule appointments, process medication refills, and submit prior authorizations
Document patient interactions accurately in the electronic health record (EHR)
Conduct outreach to patients and pharmacies to support care coordination and medication safety
Requirements
AAS in Nursing required; BSN preferred
Active New York State RN license with BCLS, Infection Control, and Child Abuse certifications
3+ years of RN experience in ambulatory, medical/surgical, ER, or physician office settings
Experience with telephone triage and familiarity with EHR systems (EPIC preferred)
Strong communication, clinical judgment, and computer skills; bilingual Spanish preferred
Compensation $76,000 - $82,000
