Key Responsibilities

• Conduct thorough assessments of individuals' needs and qualifications using care management tools and data/information audit
• Identify high-risk factors and service needs that may impact individual outcomes and care planning, referring to clinical case management or crisis intervention as necessary
• Facilitate and implement assigned care plan activities, monitoring progress and adjusting plans as needed
• Collaborate with caseworkers, supervisors, Clinical Directors, and other health programs to overcome barriers and achieve objectives
• Present cases at case conferences to obtain multidisciplinary review and achieve optimal results
• Identify and escalate quality of care issues through established channels
• Utilize discussion skills to secure appropriate choices and services necessary to meet individuals' benefits and healthcare needs
• Employ influencing/ motivational interviewing skills to ensure maximum individual engagement and promote lifestyle/behavior changes to achieve optimal health
• Provide education, information, and support to empower individuals to make ongoing independent clinical or healthy lifestyle decisions
• Assist individuals in actively and knowledgeably participating with their healthcare provider in healthcare direction

Essential Qualifications

• 2+ years of experience in behavioral health or social services
• 2+ years of experience in Microsoft Office applications (Word, Excel, Outlook)
• Bachelor's degree or non-licensed master's level clinician with a degree in behavioral health or human services (psychology, social work, marriage and family therapy, counseling) or equivalent experience

Preferred Qualifications

• Case management and discharge planning experience
• Managed Care experience
