Job Summary
As a Remote RN Case Manager, you will be responsible for:
- Patient Assessments: Perform comprehensive patient assessments and develop individualized plans of care
- Care Coordination: Collaborate with the care team to coordinate patient care and services
- Patient Education: Teach patients and families about self-care techniques and promote health and independence
- Supervision: Supervise LPNs and HHAs to ensure high-quality patient care
- Documentation: Complete accurate and timely documentation
- Communication: Regularly communicate patient progress to the clinical manager and care team
- Staff Education: Plan and provide staff education to ensure ongoing learning and development
- On-Call Responsibilities: Participate in on-call responsibilities and provide services to patients and families as needed
- Clinical Development: Participate in clinical development and continuing education programs to stay current in your field
Requirements
To be successful as a Remote RN Case Manager with Amedisys, you will need:
- RN License: Current RN license in the state(s) you are assigned to work
- CPR Certification: Current CPR certification
Clinical Experience:
- One (1+) year of clinical experience as a Registered Nurse (RN). If less than 1 year clinical experience as a RN, candidate must be approved by VP Clinical.*
- If less than 6 months clinical experience as a RN, candidate must participate in RN Intern program.
- Driver's License and Transportation: Valid driver's license, reliable transportation, and liability insurance
