Leads a care team as a clinic case manager, providing clear instructions to team members and delegating tasks appropriately. Accepts accountability for staff time management in the assigned clinic, allowing staff to leave at the direction of the case manager. Utilizes efficient and effective communication with all team members to ensure well-organized quality care. Ensures education and instructions are clear to minimize follow-up calls and inquiries.
