The Manager ACO Growth will drive membership growth for our Accountable Care Organization (ACO)solution. This role will play a critical role in orchestrating and executing our ACO growth strategy in close collaboration with the growth, ACO, market enablement, and market leadership teams. The primary responsibilities include but are not limited to identifying priority markets, medical groups, and SNF operators for growth, building, managing, and communicating our ACO growth pipeline in partnership with the Longevity growth team, and working closely with prospective and existing partners to participate in our ACO model. This role is focused on building new and expanding existing strategic partnerships with SNF operators and Increasing penetration of ACO enrollment in our senior living communities.
Requirements
Role is fully remote with up to 50% travel (mix of day & occasional overnight as needed)
Key Tasks and Responsibilities:
Business Development & Growth:
- Identify and pursue new opportunities to increase ACO Participation leadingbusiness development activities including but not limited to prospecting, outreach, and solution presentations.
- Partner with the growth, ACO, and markets team to develop a targeted growth pipeline to help Longevity achieve our growth strategy and enterprise membership goals.
- Coordinate with the growth and markets team to integrate our ACO solution into both new and existing relationships.
- Develop and execute growth strategies by leveraging existing provider networks and SNF partnerships.
- Achieve enrollment targets through proactive outreach and relationship management
- Assess SNF partner needs and present tailored solutions that align with Longevity Health’s clinical and financial goals.
- Facilitate partnership discussions and agreements the support shared success in Medicare Advantage (ISNP), ACO and other value-based arrangements.
Provider, Facility, and Community Engagement:
- Build strong, consultative relationships with medical groups, long-term care facilities, and senior living communities.
- Present program benefits and value propositions to decision-makers to drive adoption to both facilities and members/beneficiaries.
- Build trusted relationships with SNF operators, post-acute care leaders, and senior living executives to identify mutually beneficial opportunities
Market Intelligence & Positioning:
- Conduct competitive analysis and market assessments to identify growth opportunities.
- Design and continually refine our ACO and integrated ISNP value proposition and positioning based on market feedback to enhance the attractiveness of our solution.
- Position our integrated ACO/ISNP Programs as market-leading solutions for value-based care.
Performance & Pipeline Management:
- Track and report on growth metrics, pipeline progress, and conversion rates.
- Collaborate with internal teams to ensure smooth onboarding and implementation for new partners.
Compliance & Reporting:
- Maintain adherence to regulatory requirements while driving growth initiatives.
- Maintain accurate and timely records of business development activities using CRM tools.
- Provide leadership with clear, actionable insights on performance and market trends.
Supervisory Responsibilities:
- There are no supervisory responsibilities for this position.
Credentials & Coverage:
- Valid state driver’s license with a good driving record and proof of automobile insurance per minimum required by home state.
- Auto insurance coverage as required by your state of residence. New employees will be responsible for providing proof of coverage prior to hire and on an ongoing basis.
Education and Training:
- Bachelor’s degree in healthcare administration, business, or related field. Exceptional communication, negotiation, and presentation skills.
- Highly motivated, self-starter with demonstrated ability to win.
- Demonstrates superior tenacity and resiliency in their approach to achieving goals.
- Ability to analyze data and translate insights into growth strategies.
- Proficiency in Microsoft Office and CRM/project management systems.
Knowledge and Experience:
- 3–5 years of experience in sales, business development, or growth roles within healthcare , preferably within SNFs, post-acute care or physician providers.
- Proven track record of driving provider engagement and network expansion.
- Strong understanding of Sales Process, ACO models, ISNP programs, risk-based contracting, and population health strategies/post-acute payment models.
- Understanding of skilled nursing operations, post-acute care pathways, and value-based care models.
- Clinical, financial and operational acumen in healthcare delivery.
