About the Role
As a Medical Social Worker with CenterWell Home Health, you'll play a vital role in our interdisciplinary care team, working closely with patients, families, and healthcare professionals to assess and address the social and emotional needs of our patients. Your expertise will help us develop personalized plans of care that promote optimal health outcomes, independence, and quality of life.
Key Responsibilities
- Assess patients' social and emotional states, identifying needs and developing plans to address them.
- Evaluate the relationships between patients' medical and nursing needs, financial resources, and available community resources.
- Provide guidance and support to patients and families, helping them access community resources and services that facilitate recovery and well-being.
- Collaborate with physicians and other healthcare team members to understand the social and emotional factors influencing patients' health and develop effective plans of care.
- Participate in care coordination activities, acting as a resource to other team members and contributing to the identification and resolution of patient needs.
- Supervise and instruct Social Work Assistants (BSW) to ensure compliance with company, government, and professional standards.
- Maintain accurate and timely documentation, including case conferences, patient/physician community contacts, visit reports, and progress notes.
- Participate in discharge planning, ensuring a smooth transition for patients as they move through the healthcare continuum.
Essential Qualifications
- Master's or doctoral degree from a school of social work accredited by the Council on Social Work Education.
- Current social work licensure in the state of Washington.
- A valid driver's license, auto insurance, and reliable transportation.
- Proof of current CPR certification.
- Minimum of one year of experience as a social worker in a healthcare setting, home health, or hospice.
- Knowledge of community resources and ability to assist with discharge planning needs.
- Excellent oral and written communication and interpersonal skills.
- Ability to read, write, and speak fluent English.
- Knowledge of medications and their correct administration.
Preferred Qualifications
- Experience working in home health or hospice settings.
- Knowledge of state and federal programs related to healthcare and social services.
- Certifications or specialized training in areas such as palliative care or gerontology.
Strong organizational and time management skills, with the ability to prioritize tasks and function under stressful situations.

Compensation and Perks
We're offering a competitive compensation range of $52,500 - $72,225 per year for this per diem role, depending on your experience and qualifications. As a per diem employee, you'll have the flexibility to choose your schedule and work hours, allowing you to balance your work and personal life.
