We are seeking a dedicated and well organized Certified Medical or Certified Nursing Assistant to join our team. In this role, you will be responsible for providing exceptional customer service and support to our patients and clients. You would be responsible for a roster of patients ensuring monthly performance goals are met.
The role varies between clinical and administrative tasks as follows:
Serve as the contact point, advocate, and informational resource for patients, care team, family/caregiver(s), payers, and community resources Identify care needs for patients with high-risk conditions and communicates daily to the care team.
Provide ongoing contact with patients
Contact facilities to review patient charts, reconcile medications and work with facility nursing staff for overall progress updates and coordination.
As directed by care team, educate patients and family/caregiver(s) about relevant information or community resources
Verify patients' information, upkeep information and medical records accurate in the EHR Scheduling appointments
Authorizing prescription refills as directed by care team
Process orders as directed by the team
Plans and manages patient care according to each patient's needs
Interviews patients and records
Clinical record management; ensure patient charts are up to date with medications, documentation, orders, etcs.
Provide support to Physicians and Medical Providers as needed.
Review lab results and communicate with medical providers as well as provide updates to family members and/or patient
Performance Requirements:
Ability to work productively, either independently or with other individuals
Have a reliable computer or laptop
Highly organized with the ability to keep accurate notes and records
Capability to effectively communicate with patients, providers, family members to implement successful care plans and strategies
Demonstrate a positive attitude and respectful, professional customer service
Acknowledge patient’s rights on confidentiality issues and follow HIPAA guidelines and regulations Knowledge of health care practices, insurance and billing
Proven experience as Medical Assistant or LPN or similar role
Ability to undertake a variety of diverse care tasks and work in a multidisciplinary team
Understanding of confidentiality obligations and nursing best practices
Excellent knowledge of medical and hospital terminology
Highly proficient with computers, Microsoft Office and navigating EMR's.
Excellent communication and interpersonal skills
Compassionate and able to handle a busy working environment
Successful completion of Medical Assistant or Nursing program is a must
CMA or CNA license required.
Job Type: Full-time
Pay: $22.00 - $24.00 per hour
